Teacher-Child Ratio — Staff stated they were out of ratio when they were in the playground with more than 24 children and 2 staff. LPA observed staff juggling around to cover ratios. Record Review showed facility was out of ratio between Feb 2-6, 2026. This poses a potential risk to health and safety of children in care
Reporting Requirements — Based on interview and record review there was an outbreak of a communicable disease and children’s authorized representatives were not notified via telephone, email or mail. This poses a potential risk to health and safety of the children in care.
Reporting Requirements — Based on interview, the facility did not report to the Department of a communicable disease outbreak at the facility on the week of Dec 17, 2024. This poses as a potential risk to health, safely of children in care.
Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 requirement. Assistant Director confirmed that the facility has not conducted an emergency drill within the past six months, the last disaster drill was conducted on 05/18/2023. which posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 5 staff members. Staff #1 and Staff #2 did not have immunization record on file. Staff #3 did not have record of Tdap, which posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 1 requirement. Assistant Director confirmed that the previous Director has separated from the company since 08/2023 and facility has not updated the Director's information, which poses a potential health, safety or personal rights risk to persons in care.
