Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 requirement. Assistant Director confirmed that the facility has not conducted an emergency drill within the past six months, the last disaster drill was conducted on 05/18/2023. which posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 5 staff members. Staff #1 did not have immunization record on file. Staff #2 and Staff #3 did not have record of MMR, which posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above in 1 out of 1 requirement. Assistant Director confirmed that the previous Director has separated from the company since 08/2023 and facility has not updated the Director's information, which poses a potential health, safety or personal rights risk to persons in care.
