Licensee admitted the infant sleeps with two (2) blankets, which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on LPA observation, the licensee did not comply with the section cited above in which the LPA observed a open area located between the Play Room and the Dining Room with exposed cables. Outdoors six (6) 2 x 4 wood against the left side yard, wood paneling around the deck that is in disrepair behind the children's outdoor play set, accessible staircase leading to the deck, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on LPA observation and record review, the licensee did not comply with the section cited above by not renewing her expired Mandated Reporter Training which expired on 03/14/2025, which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on LPA observation and licensee interview, the licensee did not comply with the section cited above. At 9:00 AM upon arrival LPA observed door to off-limit area described on LIC 999 Sketch as storage accessible to children in care. Additionally, the licensee admitted to putting the infant down for nap time in an off-limit area which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on licensee and(P1) for (C1) interview and record review, the licensee did not comply with the section cited above in two (2) out of five (5) children's records reviewed which poses a potential health, safety or personal rights risk to persons in care.
