LPA observed a closet with cleaning supplies and chemicals located in room 6 (kitchen) that was unlocked making items accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed preschool and toddler children were commingling in room 2 upon arrival. LPA observed during outdoor play, all children were in the preschool playground which poses/posed a potential health, safety or personal rights risk to persons in care.
