Preschool Program with Toddler Component — Based on observation, the licensee did not comply with the section cited above as 5 children enrolled in the toddler program were commingled with 4 children enrolled in the preschool program inside of classroom 4, which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above as LPA observed that a day-care child was left unattended while in the restroom and LPA observed that staff were not providing appropriate supervision while children were napping as staff were placed between classrooms and were unable to supervise both classrooms at once which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation, interview, and record review, the licensee did not comply with the section cited above as a child was left alone inside the classroom for approximately three minutes which poses an immediate health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above as LPA observed that one of the containers used for solid waste does not have a tight fitting cover which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that the changing table in Classroom #3, which services children ages 18-36 months, was not placed within arms reach of a sink which poses a potential health, safety or personal rights risk to persons in care.

Infant Needs and Services Plan — Based on interview and record review, the licensee did not comply with the section cited above as C1 was missing proof of the Infant Needs and Services Plan which Director stated was never completed for C1 which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C3, C5, and C6 were missing proof of the physician’s report which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C3 and C6 were missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of measles and pertussis immunizations which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, the licensee did not comply with the section cited above as LPAs observed in the small playground a Little Tikes cart with two visible split open tires that children had access to which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review and interview, the licensee did not comply with the section cited above as LPAs observed that C7 was missing proof of the Physician’s Report which poses a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above as LPAs observed that S1 and S3 were missing proof of current Mandated Reporter Child Care Providers training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above as LPAs observed that S5 was missing proof of the Health Screening Report (LIC503) which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, LPAs observed that S1-S6 were missing proof of yearly influenza immunization and S5 was missing proof of the measles and pertussis immunizations which poses a potential health, safety or personal rights risk to persons in care.
