Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above as the facility stored cleaning solutions under the classroom's sink that was not latched and stored hand sanitizer in areas accessible to children, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on records review, the licensee did not comply with the section cited above as staff #3 did not have a completed health screening with TB screening results and additionally, staff #3 did not have immunizations on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the facility did not comply with the section cited above. All three staff member's Mandated Reporter Training expired on 11/2023, staff must renew mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the facility did not comply with the section cited above. Record review indicated that Child #2 and Child #3 did not have proof of tuberculosis screening/results on file which posed a potential health, safety, or personal rights risk to persons in care.
