Infant Safe Sleep (b)Cribs or play yards shall be free from all loose articles and objects. This was not met as evidenced by: Upon LPA's arrival, LPA noted an infant sleeping in a crib with a fitted sheet and another loose pad under them. The infant had a blanket over the top half of their body. Immediately, licensee removed the blanket and the infant woke up. This poses an immediate health, safety or personal rights risk to persons in care. This is a repeat citation, and will come with a $250 civil penalty.

For a Small FCCH, the max number of children for whom care may be provided at any one time, including children under age 10 who reside at licensee's home, shall be one of the following:(3)More than 6 and up to 8 children, without an additional adult attendant, only if the criteria in Section 1597.44 of the Health and Safety Code are met. This was not met as evidenced by: Upon arriving, LPA noted licensee caring for 8 children total, 6 infants and 2 preschool age children, by herself. This poses an immediate health and safety risk to children in care.

Family home — home free of safety hazards — Based on observation, LPA found Lysol cleaner for toilets in the on-limits restroom to the right side of the toilet used by daycare children. In the mirror storage, there also a pair of tweezers as well as a spray that should be kept out of reach of children. In the bathtub, licensee is reminded to keep chemicals high up instead of on the bottom part of the shower. This poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in 4 out of 4 infants. Upon arrival, LPA observed 4 out of 4 infants sleeping in cribs, with multiple blankets under them, stuffed toys and pillows besides them, and other forms of coverings over them. This poses an immediate health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity — Based on LPA's observation, the licensee did not comply with the section cited above. LPA observed 4 infants, and 6 more children over the age of 2, but under the age of 6 in care. Licensee was by herself, with no assistant present. This poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, C5, who is 9 months old, did not have an LIC9227 on file. This poses/posed a potential health, safety or personal rights risk to persons in care. This is a repeat citation, and will come with a $250 civil penalty.

(a) The licensee shall maintain, in each child's record, the signed and dated notice form required in Section 102419(d). (1) The licensee shall keep the signed and dated notice form for at least three years following termination of service to the child. This was not met as evidenced by: LPA requested children's files from the May 2026 inspection; however, licensee was unable to provide those files, and stated they no longer attend and are at school. This poeses a potential risk to the health, safety or personal rights risks to person in care.

Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. Although licensee had a disaster drill log on file, a drill was not completed since 10-31-2025, placing licensee over the 6 months, and resulting in non-compliance with Title 22. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. 1 out of 4 infants were observed with a clipped on pacifier. LPA was unable to determine if the infant had this on while sleeping, or if it was clipped on once they woke. In either case, this poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on LPA observation, the licensee did not comply with the section cited above. Licensee had an adult come to the home who did not have a clearance nor immunizations on file. However, the adult was seen holding a daycare child, who was not her own child. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. Licensee did not have an updated roster on file ready for LPA review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Operation of a Family Child Care Home — Based on record review, 4 out of 5 children's files did not have signatures on the LIC282. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, C6 did not have an LIC9227 on file, and licensee admitted to the infant being about 4 months old. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review and licensees own admission, the licensee did not comply with the section cited above. No 15 minute sleep logs were available for review, although LPA observed 4 infants sleeping in cribs during todays inspection. This poses/posed a potential health, safety or personal rights risk to persons in care.
