(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This is not met as evidenced by: On 2/12/26, C1 was unsupervised in the classroom for approximately 1 minute. Staff did not observe C1 enter the classroom by themself which poses a potential risk to the health and safety or personal rights of children in care.
Responsibility for Providing Care and Supervision — child C1 was found outside of the outdoor preschool play area fencing but within the secondary fence based on staff admission on 1/30/25. The child was within audible range. This poses a potential health and safety risk to the child in care.
Food Service — staff interviewed indicated that milk was not served on one day in mid January 2025 dues to milk being spoiled and the program ran out of fresh fruit. This poses a potential health and safety risk to the children in care.
Reporting Requirements — the incident occurrence date was 1/30/25 and the department was notified with a unusual incident report on 02/18/2025. This poses a potential health and safety risk to the children in care.
